The initial new patient consultation will include:

• A complete medical and social history

• Review of patient questionnaire

• Review of past medical records, if necessary

• Physical exam

• Development of an action plan

• Discussion of concerns and expectations

• Blood work or other studies may be ordered at this initial visit

After the first visit, additional visits are generally 30 minutes. At these follow-up visits, we may review laboratory results and the treatment plan, evaluate progress and make changes in treatment plan if necessary.  New problems or issues may also be discussed.

A general wellness visit is available for families of established patients.  This is for general health guidance, with the focus on maximizing the health and nutrition of all children within the family.  This visit is offered at a discounted rate, as the ultimate goal is to keep all children well, and prevent them from needing more individualized attention.  If, however, other children have significant health issues, this visit may not be sufficient and a complete evaluation may be needed to provide the best, individualized care.

Fees:

• Initial appointment:  $250
• Subsequent 30 minute appointment:  $100
• Wellness Visit (sibling or family visit of established patient):  $150

You may pay by Visa, MC, Discovery or by check.  Payment by credit card is preferred for the first visit.

*Please realize that Saturday appointments are in high demand.  Consequently, there will be a $50 fee obtained to hold Saturday appointments which will be applied to the cost of the total office visit.  No shows or cancellations within 1 week of the office visit will result in forfeiture of this $50 fee.
