How to Prepare

Prior the initial office visit, you will be sent several forms to complete. Please read them over in detail as some may take time to prepare.

 

Patient Questionnaire & Diet Diary

During the days before the initial visit, please pay attention to your child’s diet and keep a 3-day diet diary. Be sure to thoroughly complete the  patient questionnaire and bring it to the first visit. This will provide Dr. Erlich a tremendous amount of information and save valuable time during the visit so you can have a more complete discussion and education.


Medical Records

If your child has a complicated medical history that cannot be adequately conveyed on the questionnaire, please provide medical records to help clarify this information. You can request your medical records directly from your pediatrician and bring them to your initial visit. This will ensure that we have your records at the time of the visit. Please understand that your pediatrician’s office is busy and may need adequate notice before providing you with the records. For this reason, request these records at least 2 weeks prior to your initial visit to Healing the Whole Child.

The Initial Visit

What to Bring

Please make sure all forms are completed prior to your initial visit and that you have any additional medical records and growth charts in-hand.

 
  • Agreement Form

  • Patient Questionnaire

  • Patient Information Form

  • 3-Day Diet Diary

  • Any medications and supplements that the patient is taking - please bring them in their original containers

  • Growth Charts from the pediatrician’s office (weight, height and head circumference)

  • Other relevant medical records i.e. lab reports or consultations from specialists

The Initial Consult

Cost: $525

Payment by credit card or cash is preferred. We accept Visa, MC, AMEX, and Discovery.

There is a non-refundable $50 deposit required to hold your appointment and will be applied to the cost of the total office visit. No shows or cancellations within 2 weeks of the office visit will result in forfeiture of this $50 fee. If an appointment needs to be rescheduled, as long as notification is received more than 1 week from the scheduled date, the deposit will be applied to the new appointment date.

 
  • Review of medical and social history

  • Review of the patient questionnaire

  • Review of medical records, if necessary

  • Physical exam

  • Development of an action plan

  • Discussion of concerns and expectations

  • Blood work or other studies may be ordered at the initial visit (additional fees may apply)

  • Initial visits are typically 1.5 - 2 hours

Follow-Ups & Wellness Visits

Follow-Up Appointments

Cost: $250
Phone Consultation: $60 per 30 minutes

 

After the initial visit, a follow-up appointment is usually recommended, especially if lab tests are ordered. This visit will usually be one month after the initial visit to allow time for the tests to be done and results to be processed. This is also a good time to evaluate the effects of any dietary changes. Follow-up visits are typically 1 hour long.

Future visits will be determined by your child’s response to the recommendations and the complexity, or chronicity, of your child’s issues.

Ultimately, I hope to see you and your child once a year for routine wellness visits.

Wellness Visits

Cost: $300

 

A general wellness visit is available for well siblings of established patients. This is for general health guidance, with the focus on maximizing the health and nutrition of all children within the family. This visit is offered at a discounted rate, as the ultimate goal is to keep all children well, and prevent them from needing more individualized attention. If, however, other children have significant health issues, this visit may not be sufficient and a complete evaluation may be needed to provide the best, individualized care.
